2.) Must have BLS and/or ACLS cert

or already being the process of attaining an Arizona medical license

Must have active DEA at time of start date
5.) Must be accountable,
professional and in-sync with credentialing
Schedule:
7on/7off
12 hour day shift/7am-7pm
Open 15+ shifts per month

Credentialing takes: 60 days, all
depends on how fast a provider can get paperwork completed

6 months- ongoing

Age range - All adult, no peds. All 18 plus
Procedures - NONE
Admissions per day - 0-2 admits per day. May start with
0-2 new patients in the AM
